The Diabetes Center of Excellence (DCOE)

The Diabetes Center of Excellence (DCOE) is an innovative, evidence-based program designed to transform the lives of ‘real’ people and improve health care outcomes. With a focus on the whole person-centered approach, we deliver culturally appropriate, community based, educational services across the diabetes care educational continuum. 

Our mission is to improve the health and well-being of underserved, vulnerable populations by providing programs and services that educate and activate participants to achieve optimal health and well-being.
