HCD International uses trained educators to facilitate a six-week diabetes self-management education workshop using an evidence-based curriculum called the Diabetes Empowerment Education Program (DEEP). Our diabetes self-management education program aims to improve health outcomes and reduce issues of health disparities among vulnerable populations with diabetes through delivering interactive and engaging workshops that teach people who have diabetes to manage their disease properly. HCD International has partnered with local providers,community-based organizations, and key healthcare stakeholders to deliver an interactive evidence-based program that will improve clinical outcomes in vulnerable populations.
Objectives
The overall objectives of our program are to teach self-monitoring, healthy lifestyle behaviors, self-care behaviors, informed decision-making, problem-solving, and active collaboration with the health care team to improve clinical outcomes, health status, and quality of life.
Participants learn how to self-manage their diabetes for a better quality of life. The classes help patients understand diabetes and its risks as well as the importance of diet, exercise, keeping regular physician exams, receiving annual foot and eye exams, managing medications and much more. Overall, it teaches participants how to live healthier and have a better quality of life.
Classes are conveniently held in the community setting at host sites such as at provider’s offices, residential living facilities, community recreation centers, senior centers, faith-based organizations, and many other community organizations. Our classes are now being held in the state of Maryland and Pennsylvania.
